The purpose of this instruction is to improve the control of records storage.

Records Pick-Up Receipt
ULV Record Storage Label
1.0 Requestor completes the ULV Record Storage Label in permanent marker for each box and places it inside a sheet protector and attaches it to the front of the carton:
1.3 Description of contents (i.e. – Vendor Files – 2005-2006 – A-F)
1.4 Date (to storage)
1.5 Destroy date
1.6 Box # ___ of ___
2.0 Requestor fills out Records Pick-Up Receipt.
2.1 Make (2) copies
3.0 Requestor emails the purchasing department to arrange pick-up
4.0 Purchasing signs both copies of the Records Pick-up Receipt then leaves one with the requestor
5.0 Purchasing places cartons in storage.
6.0 Purchasing enters placement of cartons into inventory.
